To schedule a 15-minute video or phone call with the Director:

1. Submit the completed intake form
2. You will receive an email confirmation that your intake form was received.
3. A staff member will contact you within 48-hours to confirm your appointment and send you the conference details.

What to expect at the call:

– Discuss the brief history and description of your concerns for your child
– Review the services or programs the child currently receives
– Determine the appropriateness of our program for your child.
– Receive specific answers to your child’s unique circumstances
